Updated Personal Information

During the term of this Agreement, Client agrees to provide StemCyte with current and updated information to include: name, address, telephone number, email address and authorized credit card information. This information may be provided on the StemCyte website or by mail.

Monthly payment on these installment payment plans, not including any earned discounts or incurred late charges that will alter these payments, is as follows:

Payment Interruption

• In the event that payment is rejected for any reason, including but not limited to expiration of authorized credit card, charges on card exceed credit limit, or Client instructs the card issuer to reject payment, then:
• Client will be notified of the payment interruption within 30 days after the payment was not received. Client agrees to provide StemCyte  with a substitute card within another 30 days of that notice, with authorization to collect past due amount and future amounts as agreed.

Default

• If the past due amount is not collected within 60 days since the payment interruption, the Client agrees that the account is in default, and the entire balance is due on the account and payable in full within another 30 days, or 90 days since payment interruption, or will be subject to a 25% penalty on the balance due within a renewed payment plan.
• If, upon final default, the balance due is either not paid in full by Client or not placed within a new payment plan, then StemCyte is authorized, without further notice, to use or dispose of the Client's cord blood unit in its sole discretion and Client shall forfeit all rights and title to the unit regardless of the amounts previously paid.

Extended Payment Plans

• 6, 12, 18 Month No Interest Plans, No Credit Check, payments made directly to StemCyte using your approved Debit or Credit Card

• 24, 36, 48 Month Payment Plans at 11.90% Fixed Interest. Subject to credit approval by CareCredit.

• Extended payment plan options are subject to credit approval by GE Capital Consumer Card Co.  Applies to purchases made on a CareCredit consumer credit card account.
